Suggestions for a disabled person to lose weight?

I am disabled in a way that I end up laying down, A LOT. And if I'm not laying down I tend to be sitting. I can sometimes walk but not very much without severe pain. I have been trying to come up with ways to lose weight but so far I have no idea what to do other than fixing my diet, which I've been working on and seen very miniscule improvement. Does anybody have any ideas on what can help me? PT and OT aren't options because I used them before and, since they weren't really helping with anything, my insurance now refuses to cover it.
