Struggling to diet after years of yo-yo'ing

I'm currently 350lb, the most I've ever weighed in my life. In my early 20s I'd peak at 340lb and fall to my lowest of 276lb after 3/4 months of calorie counting and exercising.

I just have no motivation or will power anymore. I spent the last couple years in hospitality where I was 276lb but incredibly fit working in a pub 14/16 hours a day, but after a foot injury that made me lose my career, ive gained 6 stone (84lb) in 1.5 years.

I've got no motivation to diet and exercise anymore, as the progress is so minimal compared to my early 20s, quitting smoking, and the constant need to eat even when I'm full.

Has anyone hit this point, and how did they break through it? I don't want to be this size, but I've struggled with my weight my entire life and at this rate it's either gunna kill me or give me my own TV series.
