Obesity as an issue of Muscle, not Fat?

I just watched this very compelling TED talk by Dr. Gabrielle Lyon in which she posits that obesity is just as much an issue of undeveloped/inadequate muscle as it is one of excess fat accumulation. What are people's thoughts on this? I'm struck by how so many people have gone to doctors for advice and rarely heard "lift weights" or "resistance train" as a meaningful solution (as opposed to just "count calories" or "restrict") etc – especially fascinating re: the way long-term caloric restriction effects our cognitive and metabolic functioning (I was moved by Dr. Lyon's discussion about her patient's brain looking like that of a pre-Alzheimer's patient).
