My friend is killing him self

TL;DR My best friend is heavily obese. He's been big his whole life and getting bigger as the years go by. We're in our early 30s and met at school when we were 4. His health is deteriorating rapidly and I want to help him change before it's too late.

Long version...

I met my friend at school when we were 4 years old. We were pretty good pals but lost touch when we went to separate schools a few years later. We ended up back at the same school when we were about 11 and by the time we were 14 we were best friends and have been since - we're now 33. I've never know him not to be a big guy. When we were in our teens and 20s we partied hard. We both drunk heavily and ate a lot a junk food. He's a seriously funny and social guy. He has loads of friends and is very well liked by almost everyone who meets him.

In the early years he was big but I wasn't. In fact I was super skinny. But in my early 20s the lifestyle caught up with me and I got pretty big too. It was only last year I lost the weight and got back to normal (big thanks to this sub).

My friend is now 322 lbs and is about 5' 10". I put these values in the NHS BMI calculator and it was literally off the scale. This is the heaviest he has been. He looks grey and sunken. He can't walk very fast. He's developed diabetes and high blood pressure. He's been hospitalised due to the blood pressure. Doctors are giving him serious warnings. I'm genuinely worried he'll die before hitting 40.

Six months ago I sat him down and we had a heart to heart. I told him how worried I was. I didn't preach. I asked him lots of questions and he answered that he was also worried and that he'd like to change. Five weeks later he'd lost 10 lbs. He was so pleased as was I. But he stalled and ground to a halt. There was always some event that he just needed to get out of the way so he could get back to the weight loss thing. Now all these months have passed and things are worse than they were then.

My question is does this strike a chord with anyone on here and if so what did your friends do for you that helped or what would you have liked them to do that would have helped?
