Losing weight - but with health restrictions

I need to shift a bit of belly weight - but I have a series of health issues making this difficult.

I can't cut carbs by much, I can't exercise for extended periods of time due to muscle fatigue, and I have a bad back joint/muscle making long periods on my feet painful.

Where is the best place to start? My BMI is 28, and I want to drop 3 stone/20kg, lose the gut, and ideally increase core strength (for my back), and increase upper body muscle.

Male, late 20s, currently approaching 14 stone (89kg) at 5"10 (1.77m)

Most of my family are severely overweight, to obese, I've been lucky that I've only bit a bit overweight, but I've put a stone on since the winter, so now I'm worried.
