Can't put myself in the right mindset

I am almost 28 and I currently weigh 400lbs. Over the years, I have tried different diets, but I can't stick to them for long. The longest I have stuck to something was 2 years ago when I consulted a doctor in Bumrungrad Bangkok who gave me supplements and fibers and no some protein (no carbs). It worked quite well for a bit, lost nearly 8-10 kgs in 2.5 months but then lost motivation.

I checked last year I had high BP but the meds I was prescribed, everytime I took them, my BP increased more than reducing (which I stopped taking and it eventually came down).

All the local doctors always tell me to get a sleep study done for sleep apnea but I never did, mainly cause sleep was one thing I usually didn't have an issue with and my dad got the test done was given the machine but it still didn't help him sleep longer at night or peacefully. But recently I have been sleeping a bit less, maybe 6 hrs of sleep and I have woken more than usual at night but the biggest issue is waking up with a bad headache. (Don't know if this effects anything, but I was scared to get it tested since I always sleep on my stomach or else I can't fall asleep).

I can't take 1 flight of stairs without gasping.

I have developed gout for my shitty eating habits.

One of the biggest reasons I end up having junk food is cause its the easiest to order in.

I try to put myself in a healthier mindset but I can't keep at it. Every night before going to bed, I think before getting ready for work tomorrow, I will do some light cardio or at least stretches to act get into starting some form of exercise, but I never do.

Please help.
