Desperate for help

Hi everyone, I've been lurking here for a bit and I've decided to reach out for some help. I'm 32 years old and I currently weigh 466lbs. I'm very ashamed of letting myself go like this. I know people are probably saying in their minds, "didn't you notice this way before you got to 400lbs." Of course I did, but I lost and gained, lost and gained. Last year I had a total hysterectomy due to cancer. I was 395lbs when I had my surgery July of last year. Now I'm gaining again, but quicker it seems. I've been looking up things, but it's all overwhelming and I'm really not sure how to create a good base to start from. I'd appreciate any help.
