Brother weighs 170KG and just going up, wants to change but doesn't?

Hey. My brother has been on a steady weight gain ever since he was little and now he's at the point he is 170kg. He dropped down to 150kg at one point and now he's all the way back up to heavier than before. My mom worries for his life, he can barley walk around without being insanely out of breath. He holds the weight well and doesn't look ridiculously fat and is capable of being mobile.

He really wants to help himself (he says) but isn't able to do it. I understand that losing weight is hard, the mentality is hard, especially when you're that weight but it being hard isn't an excuse. I need some advice on how people who've gone from a life threatening weight to being healthy did it, how you adjusted your mindset and how you were pushed in the right direction. How other people supported you and how you removed the addiction.

There is times where he is very motivated to start losing weight but it never follows through. He always ends up over eating, etc. All tips/advice would be very much appreciated. If you have any questions to help then feel free to ask.
