Struggling to lose any weight, in fact gaining weight

I am hoping for some advice on why I seem to be gaining weight rather than actually losing any. I am 6'2" and last time I stepped on the scales was 229lb. At one point I had gotten down to 205lb, so I can't figure out exactly why I have gained 25lb despite my best efforts.

On average I work out 5 times a week, for anywhere from 30 minutes to an hour at a time. Most of the time I will lift weights on Mon, Wed and Fri (I have no set routine, but will pretty much mix things up with body weight exercises, compound lifts etc.), then either use a treadmill for HIIT, or a longer cardio session using an elliptical on Tue and Thu, then typically some cardio again on Sunday. Usually Saturday I don't exercise, but do a decent amount of walking. I try to push myself each time I exercise, because I know that 30-60 minutes is not a huge amount of time. I have been doing this for at least a year now, even before that I would still exercise pretty regularly just a different routine.

Diet wise, I am slowly cutting out as much junk as possible, while not being miserable and denying myself everything. I could not tell you the last time I had a soda. Breakfast I usually have a cup of black coffee and bowl of cereal, lunch is usually my worst meal in that I typically don't have one, but instead will grab whatever snack I find in the kitchen, dinner is for the most part a home cooked meal that my wife and I try to balance between healthy and something our kids will eat. In the evenings, typically I won't have anything after dinner, occasionally a bowl of ice cream, but that's at most once a week on average. I don't smoke, rarely drink alcohol (talking a 6 pack of beer a month or two). I rarely feel hungry, occasionally if I eat a bowl of cereal I will feel hungry around 10am, but not always. I also rarely feel full (unless I have eaten an absurd amount of food). I would certainly not say my diet is great, but I don't know I would call it terrible either.

Most of my fat is around my stomach, so about the worst place someone would want it to be! I am still able to wear the majority of the clothes at 229lb as I did at 205lb. I typically wear a 36" pant size, but had been able to wear some 34" size for a short time. Some of the more fitted shirts I own, I won't currently wear, but the majority of shirts I wear now I have had for years.

So, what am I supposed to do? I don't have a goal weight in mind, but I also don't want to be carrying around 20lb or more than I need to and putting myself at risk of issues when I get older. I usually avoid the scale, but stepping on it the other day and seeing 229lb is pretty depressing when I was hovering around 214lb or so the last few times I had stepped on there.
